BCPS vs BCACP: which BPS credential fits your practice

BCPS and BCACP sit on the same exam frame: 150 items, 125 of them scored, in 3 hours 45 minutes, with the same $600 first-time application fee. They certify different practice. BCPS spreads its outline across many disease areas, therapeutics and professional practice; BCACP puts 79% of its outline on patient care in ambulatory settings and asks for ambulatory experience to match. Every figure below comes from the Board of Pharmacy Specialties (BPS) documents for each credential.

Holder counts are the live counters on each BPS specialty page. Results are from BPS’s 2025 continuous-testing release, which reports each specialty on its own and ranks none of them; a first-time result describes a cohort, not the exam’s difficulty. Fees: BPS dates and fees.

2Where the two outlines split

The BCPS outline has three domains of nearly equal weight. Patient Care Specialty Areas (36%) names disease areas from infectious diseases and cardiology through oncology and pediatrics. Therapeutics and Patient Management (36%) covers planning, starting and monitoring therapy. Professional Practice (28%) holds quality, evidence-based practice and practice management. The BCPS study guide breaks every one of those areas into its own page.

BCACP concentrates its weight. Under the outline that took effect in October 2025, Patient Care carries 79% and Professional Practice 21%, with no separate therapeutics domain. The patient-care share is written around ongoing care in clinics and other ambulatory settings.

For a clinic pharmacist leaning toward BCPS, the gap is breadth: inpatient areas such as critical care sit on the outline, and so does a sizable evidence share. Biostatistics and evidence-based practice alone is 7% of the BCPS outline, more than any single disease area by our estimate.

If you are weighing an inpatient specialty instead, BCPS vs BCCCP for ICU pharmacists runs the same comparison, and five BPS credentials side by side puts BCACP in the wider field.

4Asked by clinic pharmacists

Where does drug therapy sit in the BCACP outline?

Inside Patient Care. The October 2025 BCACP outline runs that domain from clinical assessment and patient-specific factors through starting, titrating, de-escalating and stopping therapy, then implementation and monitoring. BCPS gives the same kind of work its own domain, Therapeutics and Patient Management, at 36%.

Do the two exams look alike on test day?

The frame matches: 150 items with 125 scored, in 3 hours 45 minutes, per each specification. The BCPS mechanics, including forward-only navigation and the single optional break, are on the BCPS exam format page.

Which one does a PGY1 graduate working in a clinic qualify for first?

BCPS, on the day the residency is complete. BCACP follows after 2 years of ambulatory care practice, or straight away after a PGY2 in ambulatory care. BPS sets no required order between the two.

Where do the holder numbers come from?

From the counters on the BCPS page and the ambulatory care page, read on the date in the sources below. They change as pharmacists certify and lapse, so treat them as a snapshot.
