Fact check

How many questions are on the BCPS exam?

The BCPS exam has 150 multiple-choice items, and 125 of them count toward your score. The other 25 are pretest items mixed in among the scored ones, and nothing on screen tells you which is which.

Short answer

150items, 125 of them scored

150 items in total: 125 scored and 25 unscored pretest items that BPS is trialing for later forms. Because the pretest items are embedded in the exam, you answer all 150 as if each one counts. The time you get for them is on how long the BCPS exam is.

2What the 25 pretest items change

Pretest items are new questions BPS is trying out before using them as scored items on future forms (BPS Candidate’s Guide). They sit among the scored items, look like them and carry no marker. Hunting for them wastes attention: an item that feels odd is far more likely to be a hard scored item (125 of the 150 are) than a trial one.

The practical effect is on pacing. You budget time for 150 items rather than 125, and since the exam moves forward only and BPS applies no penalty for guessing, every item gets your best answer before you confirm and move on.

Item shape matters as much as the count. Most BCPS items are single-answer with four options; BPS notes that some exams may include a small share of multiple-response items (three or four picks from up to eight options) and items supplemented by an image (BPS Pharmacotherapy page). Sitting the timed BCPS mock exam at the exam’s per-item pace is the quickest way to learn what a long forward-only run feels like.

3Pretest items and counts, explained

Do the pretest items affect my score?

No. Only the 125 scored items count; the 25 pretest items are being trialed for later forms (BCPS Examination Specifications).

Can I tell which items are unscored?

No. BPS embeds them within the exam so they cannot be told apart from scored items (Candidate’s Guide).

How many scored items come from each domain?

45 from patient care specialty areas, 45 from therapeutics and patient management and 35 from professional practice, per the BPS job analysis summary. The study guide contents map each area to its page.

Is the count the same on every form?

Yes: BPS describes every BCPS form as 150 items with 25 unscored, and it equates forms so results stay comparable. How that feeds the scaled 500 is on the BCPS passing score page.
